As a member of our facility, you'll collaborate closely with our Director to spearhead the organization, coordination, and execution of our Social Services Department.

Responsibilities:
Conduct Comprehensive Interviews:
Delve into the social histories of our residents and their families.
Engage Families in Planning:
Involve residents and their families in crafting social service programs whenever feasible.
Provide Practical Solutions:
Offer assistance in addressing social and environmental challenges, such as financial aid applications, discharge planning (in collaboration with community agencies), and referrals to specialized community resources.
Collaborate Actively:
Work hand-in-hand with our administration, medical and nursing staff, as well as other relevant departments to strategize and execute social service initiatives.
Facilitate Compliance:
Participate in Facility Surveys conducted by authorized government agencies, as required or directed.
Expert Communication:
Interpret the social, psychological, and emotional needs of our residents and their families to our medical team, attending physicians, and fellow resident care team members.
Requirements:
Educational Excellence:
Possess a Bachelor's or Master's degree in Social Work.
Licensure/Certification:
Hold state licensure or certification as a Social Worker.
Healthcare
Experience:
Previous experience in a healthcare or nursing home setting is preferred.
