Palliative Care Physician

Monogram Health is seeking a board-certified Palliative Care physician in (Newark) New JerseyTreatment and oversight will occur through telehealth and locally in personThe physician will connect with CKD and ESRD patients who are appropriate for conservative careThe physician will also engage with patients who have multiple complex co-morbiditiesbe expected to manage patients who are on our conservative pathway.This job is ideal for physicians who embrace the trends of telehealth and value-based care, rewarding outcomes rather than utilizationThe physician must have experience in end-of-life issues, advocating for conservative care where appropriate and where evidence-based criteria show that dialysis will not extend the duration of lifeMonogram physicians will work remotely from home and locally in person when necessaryThis is a full-time role, 5 days a week, Monday through Friday.
Responsibilities:
Build & Manage Panel:
Collaborate with POD Medical Director to proactively identify CKD& SRD members in predefined geographic regions to build patient panelOwn all outcomes of patient panel, driving clinical and financial KPIsDrive Clinical Interventions for Patient Panel:
Treat panel of patients both in-home and virtually as necessary to drive healthcare outcomesApply evidence-based criteria in determining the appropriateness and necessity of care;in particular, in determining whether dialytic or non-dialytic care is the correct evidence-based pathwayReduce pain in patients who have selected a palliative care pathwayRound on patient panel (if admitted) in selected hospitals within a defined geographic marketReduce preventable episodes of care including hospital readmissionsRefer members to appropriate specialists to drive clinical outcomesDevelop key relationships with local providers (PCP, Nephrologists, Specialists) to improve market alignmentSupport Clinical Team:
Work with local Monogram PODs to drive KPIsParticipate in monthly sessions with the Regional Medical Director and CMO concerning KPI expectationsBackup coverage will be provided by Monogram MD team membersMD must maintain active privileges at local dialysis centers and hospitalsReview Annual Wellness Assessments to ensure proper coding and provision of medical diagnosisAssist in the training of NPs and other providersRequirements:

years treating patients wiwth CKD and ESRDDemonstrated experience applying evidence based clinical criteriaExperience in geriatrics or palliative careComfort and skill in end-of-life discussionsExperience with high-need Medicare Advantage and managed Medicaid populationsExpertise in managing medication panels for complex patientsCurrent state medical licenseWillingness to become licensed in multiple statesDoctorate degree in medicineBoard-certified in geriatric medicine or hospice and palliative medicine
